Checks will be made payable to the team and mailed to the manager or coach noted on the WAGS STAR Record. If the team does not complete the form correctly, the check will be mailed to the team’s current club.

Failure to submit the STAR card within 30 days of your last Fall 2009 match may result in your team not receiving payment.
